COVID-19: Confidential Health Survey

Early learning programs in Allegheny County have been open for a few weeks, and the Allegheny County Department of Human Services and Early Learning Resource Center Region 5 continue to collect information on COVID-19 exposure in early learning programs via a survey.

The confidential information collected will help track any potential outbreaks of COVID-19 and ensure the well-being of county residents and children. This survey should be completed for children or staff who are showing symptoms of COVID-19 or have tested positive for COVID-19.  

If there is a positive case of COVID-19 in a child or adult who has been present in your early learning program, please inform the Office of Child Development and Early Learning (OCDEL) by contacting the appropriate Regional Office of Certification, and contact the Pennsylvania Department of Health (DOH) at 1-877-724-3258 for further guidance.

OCDEL also offers guidance for providers on protocol to follow if a child becomes sick in the middle of the day or if an employee/child has tested positive for COVID-19.
