The Penn State College of Medicine is inviting the public to participate in a voluntary survey to share knowledge and perceptions about primary prevention in child care settings.

Information provided during the survey is anonymous. Participants will be offered the chance to click on a separate link to provide information to receive an incentive and request information on the Antibiotics Awareness Program (Get Smart Program). Research administrators will randomly draw ten participants to receive a $50 award and ten participants to receive a free children's book. The survey will take approximately 20 minutes.

Taking part in the research study is voluntary. Participation implies consent to participate in the research. If participants have questions regarding their rights as a research subject or concerns regarding their privacy, they may contact the research protection advocate in the HMC Human Subjects Protection Office at 717.531.5687.
